To change who receives your death benefits, you can down load and fill in a nomination form [PDF]. If you can't open the pdf form, please e-mail us on pensions@lpf.org.uk or phone on 0131 529 4638 and we will arrange to post a form out to you. The form must be printed out and returned to us by post because we need your signature.
Death benefits can be shared between more than one person. For example, you may choose to nominate your brother for 30% of the death benefit grant and your sister for the remaining 70%. Just make sure that the percentages add up to 100.
If you marry or enter into a civil partnership, the nomination form we have for you will stop being valid. You must fill out another one, even if the person you are nominating has not changed.
